Complete referral form
A self-referral is where you as parents each complete an application form to request that contact be set up at an our centre. In order for this to work well there both parties would have been agreed on dates and times of appointments.
For a self- referral to be successful, both parties must be in agreement.
Where parties are not in communication, we would be able to liaise with a supporting professional such as a family mediator or solicitor.
We will review and contact you to discuss your needs
We would arrange a meeting which can be virtual to discuss the referral and complete a contact plan and risk assessment.
We would send the contact plan and risk assessment to all parties for agreement and signature
We would arrange a tour of the Centre with the child invited along with the resident parent/carer to familiarize the child with the centre
Commence contact in line with the contact plan.
