Complete referral form
A self-referral is where you as parents each complete an application form to request that contact be set up at a our centre. In order for this to work well there both parties would have been agreed on dates and times of appointments.
For a self- referral to be successful, both parties must be in agreement.
Where parties are not in communication, we would be able to liaise with a supporting professional such as a family mediator or solicitor.
We will review and contact you to discuss your needs
We would arrange a meeting which can be virtual to discuss the referral and complete a contact plan and risk assessment.
We would send the contact plan and risk assessment to all parties for agreement and signature
We would arrange a tour of the Centre with the child invited along with the resident parent/carer to familiarize the child with the centre
Commence contact in line with the contact plan.
Professional Referrals
Complete referral form and risk assessment
Share the contact plan as agreed by all parties in the initial court hearing.
We will then review this information and get in contact with the professional to make a contact plan.
This will cover duration, frequency and outline expectations of contact, both from the family and the supervisor.
Invite child and resident parent to tour the contact centre and familiarize the child with the centre
share the contact centre agreement and obtain signatures
commence contact in line with the contact plan
share reports with the referring professional
Payment Process
We require a referral fee, which is non refundable, of £165. This is payable at the time of referral
Session fees are payable by bank transfer which needs to be made prior to the session 2 days before.
Cancellations - any appointment cancelled within 5 days will incur a 50% charge. Cancellations within 48 hours incurs a 100% charge.
The commissioner of the service is responsible for the charge irrespective of the reason for cancellation.
